HOW TO COMPLETE THE WITNESS SLIP:
- Identification – Fill in your name and contact information.
- Representation – Type “Self”
- Position – Click “PROPONENT.”
- Testimony – Select “RECORD OF APPEARANCE ONLY.”
- Check the box “I agree to the ILGA Terms of Agreement” and click “Create (Slip)” to submit.

I have read that you don’t have to be from Illinois. The opposition has slips from out of state. Please do your own research though!

I understand that selecting “RECORD OF APPEARANCE ONLY” on an Illinois witness slip means we are registering our position on a bill for the record without testifying in person, submitting written testimony, or being called to speak. It just acts as a digital show of support.
